Readme.md: Improve section (5)

Set the target lts from focal to jammy, and update the examples to show
backports from lunar rather than kinetic.

Along with this, start parameterizing the command lines to make it
clearer what to change for newer releases.

This also includes use of `pkg-versions` from ubuntu-helpers to identify
the package version to be backported. I believe this gets the right
version number however this may need further refinement to generalize
properly.
