Description of the change

Revised chapters 2, 3 & 4 to conform to Precise. Mostly changed menu names and paths.
(migrating-upgrading.xml, guide-desktop.xml, guide-default-apps.xml)
Modified entities that pertained to System and Settings Manager since they are
under different places in Applications Menu in Precise.

I have no specific opinion on moving Simple Scan under Graphics, but if we do that, we should do the same with the Saucy docs. Same with Ibus->IBus.

Duly noted. There were a few other things I encountered that I realized should also be checked in the Saucy docs. Will do that once we finish up with Precise.
